MISSION STATEMENT

To provide quality care to children and adults living with developmental disabilities in a safe, loving home environment in which they are treated with respect, compassion, dignity and provided the means, support and encouragement to reach their full potential.

HISTORY

Forgotten Angels was founded in 2001 by Renee McGuire, a Pearland business owner, to help friends who
were struggling to find appropriate services for their adult children with disabilities. Forgotten Angels Foundation provides specially designed homes and facilities, as well as recreational, educational, and vocational opportunities for more than 150 individuals with special needs.

OUR SERVICES

HOME AND COMMUNITY-BASED SERVICES (HCS)

Forgotten Angels is a private, non-profit company who provides services for individuals with intellectual and/or developmental disabilities.

Forgotten Angels offers community-based living environments for qualifying individuals and a comprehensive system of community-based services.

These Services include:

- Residential Services (Host Home Companion Care, Supported Home Living, Foster Care, and Transportation)
- Primary Care (Psychiatric On-site Clinics, On-site Laboratories)
- Consumer first Choice (CFC) and Consumer Directed Services (CDS)
- Case Management
- Specialized Therapies (Occupational Therapy, Physical Therapy, Speech Therapy)
- Dental Services
- Dietary Services
- Behavioral Support
- Nursing Services (Special Needs over 40)
- Minor Home Modifications
- Adaptive Aids
- Supported Employment
- Day Habilitation Program

How to Obtain Services:

To qualify for the HCS program, individuals must have a Determination of Mental Retardation (DMR) or a Related Condition (RC) established through a Local Intellectual and Developmental Disability Authority (LIDDA) and qualify for the Medicaid Home and Community-Based Services program. There is a waiting list for the HCS program. If you are not on the waiting list, contact your LIDDA. They will assist you in determining eligibility and completing the paperwork so that you may be placed on the waiting list. You will be contacted by the LIDDA when a position becomes available. The LIDDA will offer a list of service providers from which you can choose who to contact or visit. REMEMBER….. If you are currently receiving HCS services from a service provider and are unsatisfied with the services, contact your LIDDA to discuss transfer options. The choice is yours!
